Depende del tamaño del sitio y las ganas de BREIN de hacer un caso, dependiendo el proveedor si tu proveedor es friendly dmca, o offshare donde te pasan esos email significa que solo actuaran en caso de orden judicial. (Asi saben que es real el asunto, los mails no dicen si son o no reales).
Los casos mas extremos si llegan a quitar dominios, cerrar servidores con redadas etc, pero para la mayoria de las personas no pasa nada, a lo mucho una suspension del servidor.
Si tu sitio no es un top tier 1 (thepiratebay o similar) dudo mucho que te lleguen a esos extremos.
Ahora, BREIN no contacta en español y es una organizacion de holanda.

This causes extensive damage to rights holders for which you - and all others involved with the management of the site - are liable.
As you are operating in full anonymity, BREIN and its participating rights holders at present are unable to recover damages from you and your organization. In the interest of and on behalf of the rights holders represented by BREIN, we request that you cease and desist the abovementioned unlawful activities immediately by removing all content from the rights holders represented by BREIN or by closing down the entire site.
We also request you to immediately provide us with the geographical address of yourself and the entity or other person(s) operating The Website. The reason is straightforward and simple:
(i) you are engaged in an unlawful activity that you must cease and desist,
(ii) we must be able to hold you liable in case you continue your unlawful activities.
